The EyeMed Vision Plan provides employees who work 1,000 hours per year or more and eligible dependents the opportunity to elect vision benefits. The plan covers routine eye care, including exams and eyeglasses or contacts. Employees have the option of using an EyeMed network doctor or using a non-network doctor. In network, the member pays the co-payments as well as any expense for upgrades or additional items above the benefits of the plan. Out of network, the member pays the full cost and files a claim with EyeMed to get reimbursed for a portion of the expenses. To search for providers or access the out of network claim form, go to
