Open Enrollment

Open enrollment is your annual opportunity to make benefits changes.  Open Enrollment for the 2017/2018 plan year is 2/6/17 - 2/17/17.

New for 2017:

  • The PPO70 medical plan will no longer be available. Employees currently enrolled in the PPO70 plan will automatically be moved to the Preferred PPO plan effective 4/1.
  • The Base Vision plan will no longer be available, and only the Enhanced Vision plan will be offered with EyeMed.
    • Employees currently enrolled in the Base Vision plan will be required to elect vision during open enrollment to continue coverage. 
    • Employees currently enrolled in the Enhanced Vision plan will remain in the plan effective 4/1.
  • New prescription tiers have been added.
  • New OptumRx cards will be mailed which will include a new customer service phone number. Old cards will still work, but the new phone number will provide enhanced customer service. 
  • Telemedicine with Teladoc will be available for a $5 copay.

Failure to complete enrollment will result in losing vision coverage (if currently enrolled in the Base Vision plan), and you will not have a healthcare or dependent care flexible spending account (FSA), effective 4/1/17.

Open Enrollment 2017

Open Enrollment will be 2/6/17 - 2/17/17, and is annual opportunity to make benefits changes that will be effective 4/1/2017.

Important: You MUST process an open enrollment election if:

  • You are currently enrolled in the Base Vision plan and would like to continue vision coverage; or
  • You want to continue a flexible spending account. The FSA amounts  do not roll over from year to year.; or
  • You want to make changes to your benefits; or
  • You want to add or drop dependents; or
  • You want to add a flexible spending account

Open Enrollment will be through the ICUBA website 2/6/17 - 2/17/17.

Plan on attending one of the sessions that will be offered to learn important information about your benefit options, how to use your benefits, plan changes, and the open enrollment period.

Benefits Provider Session

Do you have questions about your plan options? Do you need help deciding which plan to choose? We will have a special Provider Information Session where you can stop by and meet with a plan representative to get one-on-one assistance with your benefit questions to help you decide which plan(s) are best for you. Representatives include: Blue Cross Blue Shield Medical, OptumRx, Humana Dental, EyeMed Vision, and Resources for Living EAP.
