The Advantica Vision Plan provides employees who work 1,000 hours per year or more and eligible dependents the opportunity to elect vision benefits. The Advantica Plan covers routine eye care, including exams and eyeglasses or contacts. Employees have the option of using an Advantica network doctor or using a non-network doctor. In network, the member pays the co-payments as well as any expense for upgrades or additional items above the benefits of the plan. Out of network, the member pays the full cost and files a claim with Advantica to get reimbursed for a portion of the expenses. Eligible dependents include:
your legal spouse
children up to age 26 (coverage ends December 31st of the year they turn 26 and they will be offered COBRA)
children beyond age 26 incapable of self-sustaining employment by reason of mental disability or physical handicap
certified domestic partners
children of certified domestic partners subject to the same eligibility standards applicable to dependents
