
CNA Insurance Company provides employees who work 1000 hours per year or more and eligible dependents the opportunity to elect long term care insurance. Long term care insurance includes coverage for a variety of support services when you are unable to take care of yourself, either temporarily or permanently. Coverage includes services such as nursing home care, home health care, assisted living facilities, adult day care, hospice care, and caregiver benefits. There are three benefit levels to choose from with varying daily nursing home maximums and lifetime maximums. There is also an additional option available called the non-forfeiture benefit account.
An enrollment form is required and is subject to CNA underwriting. The effective date of coverage will be the first of the month following CNA's processing and approval of your application.

your legal spouse
certified domestic partner
Coverage is also available for parents, grandparents, and in-laws by completing the long form application and making payment directly to CNA. Coverage for parents, grandparents and in-laws is subject to CNA underwriting approval and may not be paid through payroll deduction.
