Welcome to the Rollins College Open Enrollment Information Site for the April 2012 - March 2013 plan year. This informational site is designed to provide basic information about benefits and changes for April 2012. You will find detailed plan information under the Benefits Library and Reference Center of the ICUBA Benefits website.
You must login to the ICUBA benefits website to access the detailed plan information. All employees who enrolled or made changes online in previous years have a user name and password. There is an option to reset your password if you do not remember it. You will need the company key, ICUBA.
Please read this page in its entirety for important information
Most benefit elections will carry over from 2011-2012. You do not need to enroll online unless you want to:
- Elect a flexible spending account for April 2012-March 2013
- Change any existing elections (includes changes and new enrollments)
If you do not participate in online enrollment you will not have a flexible spending account for the April 2012-March 2013 benefits plan year. It is a good idea to check life insurance beneficiary information each year (login required). Please see the
Open Enrollment Guidebook and visit the
ICUBA Benefits website for plan and rate details.
Forms required & due to HR by February 17th in addition to online enrollment:
- New enrollments - Long Term Care Insurance
- New enrollments/changes over the Guaranteed Issue Amount - Voluntary Life
It is recommended that you attend an Open Enrollment Meeting and spend time early on reviewing the plan options and making decisions that best fit the needs of you and your family. Once your decisions are made, you will need to proceed through the Open Enrollment process on the ICUBA Benefits site to complete your selections. The ICUBA Benefits Website will be available for Open Enrollment through 5:00 pm, February 17, 2012. No changes will be accepted after February 17, 2012.
RECOMMENDED STEPS FOR OPEN ENROLLMENT COMPLETION
- Review this Rollins College Open Enrollment Information Website for basic information
- Login to the ICUBA Benefits Website (Company key for password reset is ICUBA)
- Review current benefitsMake decisions on all plans
- Start & complete the online open enrollment process on the ICUBA Benefits Site (must hit "I Agree" for elections to be saved)
- Review & print your confirmation (can be retrieved at a later date by logging back in to ICUBA Benefits Site)
NOTE: Elections carry forward except Flexible Spending Accounts . Only employees needing to elect flex or make changes to existing elections need to go through online enrollment. Login is still required to review the new rate and plan design information under the Benefits Library.
IMPORTANT INFORMATION ABOUT ELECTIONS AND CHANGES
- PLANS THAT MUST BE SELECTED TO HAVE FOR APRIL 2012: This is a "positive" enrollment for flexible spending accounts (HCSA & DCSA). This means that in order to have these benefits for the new plan year of April 2012- March 2013 you must select a plan online; they will NOT roll over from last year.
- CHANGES TO HEALTH PLANS: Your current health plan election will roll over. There are some important plan benefit and rate changes. Details can be found under the Open Enrollment Guidebook on the Rollins Open Enrollment website and in the Benefits Library section of the ICUBA Benefits website.
- COLLEGE LIFE BENEFICIARY REQUIRED FOR ALL: ALL full time faculty and staff must have beneficiary information online for the free basic college life insurance. It is a good idea to check this each year. You will need beneficiary name, SSN, DOB and address/phone. If you are electing voluntary life you must also enter beneficiary information online.
- FORMS REQUIRED: If you are not currently enrolled in voluntary life or long-term care insurance and wish to elect one of these benefits, you must make your selection online. In addition, if you are enrolling for the first time or making changes to the voluntary life (above the guaranteed issue), there are additional paper forms to be completed found in the HR office or ICUBA Benefits website.
- DOMESTIC PARTNER BENEFITS: You may use the online enrollment process to elect or change domestic partner benefits. However, you must be sure the Human Resources Department has the Domestic Partner Affidavit on file and the proper Tax Declaration form. Please make an appointment with Jennifer Addleman if you have questions.
ADDITIONAL ASSISTANCE

OPEN ENROLLMENT CLOSES 5:00 PM, FEBRUARY 17th
All selections must be completed online and any corresponding paperwork turned in to Human Resources by no later than February 17, 2011. Selections will be effective April 1, 2012 unless underwriting is required by the insurance company (life insurance and long term care).
