Rollins Summer Camp Returning Employee Information

Please note: All of the below information are for applicants that have already applied and been hired as Summer Camp Employees. The applicant process for the 2013 Summer has been closed.

Thank you for your interest in working with the 2013 Rollins Summer Camp. Please see below for items that must be completed prior to your employment with the camp. Please plan on completing all items by May 10, 2013.

Required Paperwork:

Please see below for all employment forms that needs to be completed. These forms will be collected at the Summer Camp Administrative Offices at 422 West Fairbanks on April 17 & April 24 from 3:00pm - 4:30pm. If you are not available during these times please contact us to make alternate arrangements.

Required Forms

• Character Affidavit - This form must be notarized by a notary licensed within the State of Florida prior to submitting to the Summer Camp Office.
• Direct Deposit Request - This form must be submitted with a voided check.
• Worker's Comp Confirmation
• W4 Form
• Online Course Certificate
  • To access the Sexual Misconduct: How Teachers and Other Educators Can Protect Our Children online course, please click here. This is an online course that is required each summer. Please click here for directions on accessing the course. Once you complete the course please bring the certificate with you with your completed paperwork.

Optional Forms

• I-9 Form - This form needs to be completed every three years. If you have not updated this form in three years please submit a new form.
• Personal Information Form - This form needs to be completed if your information needs to be updated.

Fingerprinting Information:

Please note that fingerprinting cannot be completed prior to April 10, 2013.

Please see the instructions on how to schedule your fingerprinting. Please schedule the appointment between April 10 - May 10 to be eligible for the Summer Camp.

Please follow the instructions below to complete your Level 2 Background Screening:

2. Click the "SCHEDULE AN APPOINTMENT" button to create an account as a new user or login as an existing user.
3. Click the link “ I know my Fieldprint Code”
4. Enter the Code: FPRollinsCollege1 in the Fieldprint Code field.
5. Complete the Data Collection, Time and Location, Payment and Confirmation sections.
6. Bring the Live Scan Background Screening Submission Form, an I.D. and your reservation confirmation to your scheduled appointment.If you do not bring all three items you will not be able to be fingerprinted.

Live Scan Background Screening Submission Form

 During the 2010 legislative session, The Florida Legislature approved (HB 7069), a new background screening law which became effective August 1, 2010. The law requires all summer camp employees that assist more than ten (10) hours per month comply with Chapter 435 Level 2 Background Screening requirements. Level 2 Background Screening requires fingerprinting for statewide criminal history checks through Florida Department of Law Enforcement (FDLE) and national criminal history checks through the Federal Bureau of Investigation (FBI), and may include local law enforcement checks. Applicants are responsible for the total cost of completing their Level 2 Background Screening. You cannot start work at the Rollins Summer Day Camp until all results are back and cleared or until you have received an exemption from disqualification. Payment for the fingerprinting must be completed online via credit card at the time you are making the reservation. Fingerprinting that is completed prior to April 10^th, 2013 will not be valid so please be sure to schedule an appointment between April 10 and May 10.
